Lily the Pink is ontleend aan Sucking Cider Thro' a Straw, op schrift gezet door W. Freear (Published ℗ 1894). Adapted door Lee David (1891-1978) en Carey Morgan (1885-1960) als Sipping Cider Thru' a Straw (Published ℗ 1919). Lily the Pink van the Scaffold is een modernisering van een oud volksliedje getiteld Lydia Pinkham [Round No. 8360], uit het boek The American Songbag van auteur Carl Sandburg, uitgegeven door Harcourt, Brace and Company (Published ℗ 1927).
De Lily The Pink die in dit nummer bezongen wordt, heeft echt bestaan. Haar echte naam is Lydia Pinkham (1819-1883) en ze wordt bekend door het geneesmiddel Lydia Pinkham's Vegetable Compound, dat ze in de jaren 70 van de 19e eeuw op de markt brengt. Dit alcoholische middel is een medicijn tegen onder andere menstruatiepijn en overgangsverschijnselen. Hoewel het ook werkzaam is tegen rug- en nierklachten, wordt het middel verkocht als een speciaal medicijn voor vrouwen.
In 1911 verschijnt het middel ook in Nederland op de markt onder de naam Lydia E. Pinkham's Plantaardig Mengsel. Na enkele jaren jaren verdwijnt het echte weer. De werking van het middel wordt betwist en critici zien Lydia Pinkham tegenwoordig dan ook als kwakzalver. In de coupletten van "Lily The Pink" gebruiken verschillende mensen het middel, waarna er vreemde dingen met ze gebeuren. Het liedje over Lydia Pinkham heeft al een lange geschiedenis. Er zijn bronnen uit de Eerste Wereldoorlog die het liedje al beschrijven. De tekst is dan nog een beetje anders, maar het onderwerp is hetzelfde.
Tijdens carnaval 1969 wordt het nummer ook in een Nederlandse vertaling een hit. Thijs van der Molen behaalt met zijn Drinke totteme zinke de twaalfde plaats. Daarnaast staan Johnny Hoes en de Feestneuzen met het Telstar Orkest o.l.v. Jean Kraft met het nummer En we drinken tot we zinken! één weekje op nummer tweeëndertig. Het oorspronkelijke onderwerp is in deze twee liedjes losgelaten; hun teksten gaan over dronken worden.
